Client Agreement –
Functional Medicine Guidance at Optimal Fertility

 

This agreement is between you as the client and Optimal Fertility, and aims to clarify the terms of our collaboration. 

1. Purpose and Framework

You have chosen to begin a collaboration with Optimal Fertility to receive support for your hormone health, fertility, and/or general wellbeing through functional medicine guidance. All guidance is based on a holistic perspective that considers nutrition, lifestyle, nutrient status, and the body’s own signaling systems.

The guidance includes:

– Individual assessment based on your health declaration, possible test results, and consultations

– Recommendations regarding diet, lifestyle, supplements, and natural supportive interventions

– Follow-up according to the agreed-upon structure

2. Scope of Responsibility

The guidance is non-medical and does not replace care or treatment from licensed healthcare professionals. You as the client are responsible for contacting a doctor if you suspect a medical condition or if your health status changes.

Optimal Fertility assumes no medical responsibility for any changes you choose to implement based on the recommendations.

3. Testing and Supplements

Any functional tests are voluntary and paid for by you as the client. Test interpretation is based on functional medicine knowledge.

Recommended supplements are based on identified needs and are always voluntary. Optimal Fertility may in some cases receive commission on products provided through partner companies.

4. Confidentiality

All information you share during our sessions is treated confidentially and in accordance with current data protection legislation (GDPR). Your information is never shared with third parties without your consent.

 

5. Cancellations and Rescheduling

Booked appointments may be rescheduled up to 2 hours before the meeting.

 

6. Payment

Payment is made according to agreement – either the full amount at the start of the program or via a payment plan offered by Optimal Fertility.

 

7. Use of Anonymized Success Stories 

By agreeing to this contract, you give your consent for Optimal Fertility to use success stories or positive outcomes from our collaboration in communication via social media and other channels – fully anonymized and without identifying details. The purpose is to inspire and support others in similar situations. You may withdraw this consent at any time in writing.

 

8. Duration of Agreement

This agreement is valid from the first booked session and covers a 6-month program. After these 6 months, the program guarantee described in point 10 applies and is based on this agreement.

The agreement may be terminated by either party at any time, with written notice.

 

9. Shared Responsibilities in the Collaboration

A successful result requires active engagement from both parties. By entering this collaboration, both you as the client and I as the coach commit to fulfilling the following responsibilities:

Your responsibility as the client: 

You are responsible for following your individualized health plan as consistently as possible during the entire program period. This includes:

– Implementing the changes recommended regarding diet, supplements, and lifestyle

– Completing any assignments or follow-ups included in the plan

– Being honest and open in your communication about results, obstacles, and deviations

– Actively participating in booked sessions and contacting me with questions or if adjustments are needed

– Seeking medical evaluation if your health status changes, in accordance with point 2

If the responsibilities above are not fulfilled, the guarantee included in the program is automatically void.

 

My responsibility as the coach: 

I commit to:

– Providing professional and personalized support, based on my expertise in functional medicine, nutrition therapy, and hormone health

– Delivering a structured health plan, updates when needed, and clear recommendations

– Keeping agreed-upon times, responding to questions within a reasonable timeframe, and communicating any changes

– Acting with integrity, respect, and with your best interest in mind throughout our collaboration

In the event of repeated deviations from the agreed terms or lack of respect for the collaboration framework, I reserve the right to terminate the collaboration.

 

10. Program Guarantee

To help you feel fully supported, a program guarantee is included.

If you have not become pregnant within 6 months from the start of the program, I will continue working with you – three months at a time – at no additional cost until you become pregnant.

This guarantee applies as long as you have actively participated in the process in accordance with the client–coach agreement. This means that you:

– Follow your individualized health plan

– Complete the assigned tasks

– Take the recommended supplements

– Make the agreed changes to diet and lifestyle

– Use the available resources and materials included in the program

If these conditions are not fulfilled as agreed, the guarantee will automatically become void.
